When submitting your request, please provide sufficient information for us to verify your identity. Only you or an authorized agent may make requests regarding your personal information.
We respond to all verifiable requests within 45 days of receipt and may extend once by an additional 45 days with notice.

We verify requests by using the information you provide and may ask for additional documentation. We will only use your information to verify your identity and not for other purposes.
